Please clarify whether services are required for HomeHealth Visiting Nurses - Bridgton and the physical address and HCP # for that site.
Answer: The correct address for HomeHealth Visiting Nurses - Bridgton is 2 Elm St Bridgton, ME. This site does not have an HCP number.
