Question: There is a list of sites to be serviced. One is “21 Gregory Drive Suite 165, Burlington VT, 05401”. There is no such address in Burlington, Vermont, 05401. Please clarify.
Response: Corrected Address: 21 Gregory Drive Suite 165, South Burlington VT, 05403

Question: Are you asking for a Single 1 Gbps served via diverse path cabling to that address, or are you asking for two (2) 1Gbps EDIA connections, with each one on a separate path?
Response: Two (2) 1Gbps EDIA connections, with each one on a separate path.

Question: Is the intent for two (2) separate Internet Access companies/providers, or two (2) 1Gbps diverse circuits from the same Internet Provider?
Response: As long as the paths are physically diverse either would be acceptable.

Question: An "Appendix D" is mentioned in the RFP, but it does not seem to be included. I assume this is the document it is referring to?

Response: Correct, this is referring to the Healthcare Connect Fund order (47 CFR Section 54.646, 746 Appendix D; 47 CFR Section 54.646)

Question: Are you requiring a written project management and implementation plan as part of the RFP response, or would this be expected prior to the contract award/negotiation process?
Response: Prior to the contract award/negotiation process.

Question: Can you please provide the Rural Healthcare Number for the corresponding RFP?
